You may say, “Why do I need a careplan? I know what I am doing. I can care for my parents in my sleep!” This might be true. However, carepartners may not realize how withdrawn or fatigued they have become.

The Individual Client Careplans (ICC) is a personalized careplan designed to meet the needs of the care recipient and carepartner. The ICC will include one-on-one case management services: information and referral inquiries, HCBS resources, service delivery management, and carepartner support references. Prior to careplan implementation, clients must complete a virtual or in-person assessment.

A CAREPLAN:

  1. Can assist you in making decisions regarding daily tasks such as:

Meal preparation - Is the care recipient able to prepare his/her meals independently?

Housekeeping - Can the care recipient successfully maintain a clean and safe environment without assistance?

Medication Management and Doctor’s appointments:

Is the care recipient able to administer his/her medication without assistance?

Does he/she remember to take their medication?

2. Can help you identify weak areas in your current careplan, which may require a need for additional care partners or support services

3. Alleviate carepartner stress and burnout
